CYSA Membership Form #1601
Note: This form requires a wet-ink signature: This form MUST be signed by the Parent/Legal Guardian OR a Player age 18 or over.  Players who turn 18 during the seasonal year must have a new form filled out. Players who are 18 MUST sign their own form.  The wet-ink signature must be in a color ink other than black if the form is downloaded from the CYSA website.  Two copies of this form must be turned into your League/Club Registrar with the appropriate fees.  You may make a copy of the signed form and turned the original and the copy in or you may turn in two signed copies of this form.

Registration fees:

• $88 for one player in a family
• $165 for 2 players in a family
• $240 for 3 or more players in a family

Please note that cousins, friends, nieces, nephews, etc. do not qualify for the multi-player discounts.

Documentation Required for Registration

PLEASE NOTE REGISTRATION REQUIREMENTS

If you want to play in RPSL this year (2011), and you have not played in RPSL before, you must bring a birth certificate, birth registration or passport issued by an appropriate U.S. government agency.

The following documents will NOT be accepted as proof of age:

• a certificate issued by the Immigration and Naturalization Service attesting to age
• state issued identification card
• driver's licenses
• certification of an American citizen born abroad
• Hospital- or Church-issued birth certificates

A parent or legal guardian must register their children. Registration cannot be completed by someone other than a parent or legal guardian.

Incomplete registration packages will not be accepted. All documentation, including fees, birth certificates, completed registration forms, must be submitted at the same time. No exceptions.

Uniforms:
Uniforms will be sold by an independent vendor at registration, and are in addition to the registration fees.

Registration and uniforms payments must be made separately. Please be prepared to pay for each with separate forms of payment at registration.

Uniforms for Recreation (D4) players are sold by Soccer and Sports, 3774 Santa Rosa Ave., Santa Rosa, CA 95407; (707) 523-0991

Refunds:
Refund requests will be honored only if the following requirements are met -

* This is the only Refund Form RPSL will process
* Print one form per player
* Fill out form completely
* Please mail to:

* No phone calls or emails will be excepted.
* All Refund Forms must be postmarked no later than July 1st
* Anything postmarked after July 1st will not be excepted.

There are no exceptions - this form must be used, it must be completely filled out, and it must be postmarked no later than July 1st for a refund request to be honored.
